The background behind the Twin Weavers concept.

Anita Lee co-designed and implemented the Frequent Flyer Emergency Department (ED) Diversion Program at New York Presbyterian Hospital (NYP) from 2002 to 2004. The project identified patients who have visited the ED three or more times over six months for diagnoses that were non-acute and preventable. With the deployment of two Health Priority Specialists serving as health educators and Community Health Workers from a community-based social service agency in Washington Heights supporting patients in navigating the health and social service systems to mitigate barriers in access, including access to Medicaid, affordable prescription drugs, accessing preventive and primary care services at appropriate settings. The project reduced ED visits from over 700 patients by over 70% in nine months.
